Effectiveness of Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) on Post-Traumatic Growth Resulting from Intergenerational Trauma in Adolescents

Objective: The present study aimed to examine the effectiveness of trauma-focused cognitive behavioral therapy (TF-CBT) on enhancing post-traumatic growth resulting from intergenerational trauma among adolescents with post-traumatic stress disorder. Intergenerational trauma can produce extensive emotional and developmental consequences; however, some individuals may experience positive psychological changes, referred to as post-traumatic growth, during adaptation to traumatic experiences.
Methods and Materials: This quasi-experimental study employed a pretest–posttest design with a control group and a three-month follow-up assessment. The statistical population consisted of adolescents aged 12–18 years in Shiraz, Iran. Following initial screening, 60 eligible participants were selected through purposive sampling and randomly assigned using block randomization into two groups of 30 participants: a TF-CBT intervention group and a control group. The intervention was conducted based on Cohen et al.’s protocol and included eight weekly group sessions lasting 60–90 minutes. Research instruments included the PTSD Checklist and the Post-Traumatic Growth Inventory. Data were analyzed using repeated-measures analysis of covariance with SPSS version 27.
Findings: Inferential analyses indicated a significant interaction effect between time and group on post-traumatic growth (F(2,74)=3.52, p=0.035, η²=0.087), demonstrating significant differences in the pattern of changes between the intervention and control groups. Furthermore, the main effect of group remained significant after controlling for pretest scores (F(2,74)=5.37, p=0.007, η²=0.127), confirming the significant impact of TF-CBT on post-traumatic growth.
Conclusion: The findings demonstrate that trauma-focused cognitive behavioral therapy significantly enhances post-traumatic growth associated with intergenerational trauma among adolescents, with effects maintained at the three-month follow-up. Accordingly, TF-CBT can be considered an effective therapeutic and preventive approach for adolescents experiencing trauma-related psychological difficulties.
